Our specialists develop rehabilitation & nursing programs tailored to each resident’s individual needs, including physical, occupational and speech therapy.
Our intermediate, skilled nursing care include:
- Three nutritious meals and snacks program planned by a dietician
- Licensed nursing staff 24 hours-a-day

- Social services and discharge coordination

Rehabilitation
After an illness, injury or accident, some people may benefit from a short stay in a rehabilitation program.
We offer comprehensive rehabilitation services and skilled nursing services under the direction of the attending physician. During the stay, an individualized care and services plan is developed with an interdisciplinary team including the patient, family, care team, and medical providers with the goals of resuming each patient’s former lifestyles, improving quality-of-life and achieving and maintaining independence. Our specialists develop rehabilitation & nursing plans tailored to each patient’s individual needs, including physical, occupational and speech therapy staff.
Once the rehabilitation and nursing goals are achieved, or when the rehabilitation stay in coming to a conclusion, our discharge planning team can assist with setting up the needed services or details for a well-coordinated discharge. This may include coordinating home health, home care, transportation, follow-up appointments and more. We want your transition to be as smooth as possible!

Sapphire at Gresham is the first Skilled Nursing facility in Oregon to be endorsed by the state to have a ventilation assistance program.
Our newly renovated, dedicated Respiratory Care Unit provides a person-centered approach to caring for 20 residents with a team of respiratory therapists, pulmonologists, doctors, nurse practitioners, nurses and certified nursing assistants.
All staff have had specialized training approved by state board, to ensure that your loved one is receiving the best possible patient centered care possible.
